Short version: this website does not have a database of form submissions. The contact form writes an email for you in your own email app, and nothing is sent until you press send.

The contact form: what it does and what it does not do

The form asks for your name, email address, an optional organization, the topic you are interested in, and a message.

When you choose Prepare Email, the website does not upload, transmit, or store anything you have typed. Instead, it opens the email app already set up on your device with a message written out for you, addressed to the laboratory.

• Nothing you type leaves your device while you fill in the form.
• Nothing is sent unless you press send inside your own email app.
• If no email app opens, nothing has been sent — please call (762) 499-0353 instead.
• We have no record of your message unless you send that email.

The website itself keeps no list of enquiries, no account for visitors, and no copy of the form fields.

What we receive, and how we use it

We receive only what you choose to include in the email you send: your name, email address, organization if you mention one, the topic you picked, and your message.

We use that information to reply to you and to follow up on the specific enquiry you raised — a provider partnership, a question about testing, or a general request for information. We do not add you to a marketing list from this form, and we do not sell or share your details for advertising.

Information recorded automatically

Like most websites, the platform that serves this site may record standard technical details about each visit so it can keep the site running and secure — for example an IP address, browser type, the page requested, and the time of the request.

This website does not place advertising or tracking cookies of its own, and it does not run analytics tools that build a profile of individual visitors.

Third-party services

• Google Fonts. The typefaces on this site are loaded from Google’s font servers. When a page loads, Google receives that request, which includes your IP address.
• Your email provider. If you send the prepared message, it travels through and is stored by your own email provider under their privacy policy, and by ours once it arrives.

No other third-party services run on this website.

Please do not send patient or health information

This contact form and the email it prepares are not a secure or confidential channel. Please do not include patient names, medical records, test results, or details about anyone’s health.

Test results are released by the healthcare provider who ordered them, and questions about results should go to that provider. If you need to discuss a specific test or specimen, call the laboratory at (762) 499-0353 and our team will point you to the right process.

How long we keep correspondence

Email you send us is kept only as long as needed to answer your enquiry and to keep a record of our own dealings with you, after which it is deleted. Because the form itself stores nothing, there is no submission record to keep — and nothing to delete — if you never sent the email.

Your choices

If you have sent us an email and would like it removed, tell us by phone or by reply and we will delete it. There are no tracking cookies on this site to manage, and you can browse without filling in the form at all. If you would rather not use email, call us.

Children

This website is not directed at children under 13, and we do not knowingly collect information from them. If you believe a child has sent us information through this site, let us know and we will delete it.

Changes to this policy

If this policy changes, the updated version will appear on this page with a new effective date. The version above is the one in force when you are reading it.
